Ke 20.1. TEMPO klo 14.15: Healthcare and post-socialism: a case study of primary care in Poland

Post-socialist transition has open up many new spaces, where individuals and institutions form identity and seek agency. However, the Central Europe, 26 years after the collapse of Soviet Union, still struggles with the astonish amount of post-transformation slivers thrusting almost all aspects of socio-cultural and political reality. The recent two decades in Poland were a turbulent period of reforming anachronistic state-provided healthcare. In 1997 Jerzy`s Buzek government decided to implement controversial, yet profound reform, fundamentally reshaping healthcare system, clearly dysfunctional in the post-socialist context. One of the effects of this reform was the arise of a complex culture of a primary care (podstawowa opieka zdrowotna, POZ).

In the lecture I will focus my attention on primary care doctors` perspectives and experiences related to this socio-politic shift and its consequences: "freemarketization" and bureaucratization of primary care. Additionally, I will turn my attention to doctors` authority, relations of power and some of the relations with patients in the new primary care settings. Finally, I will demonstrate how a contrary medical ideologies – the "traditional", focused on patients` story and body – and – the "modern", anchored in standardized, technological regimes – clash in the contemporary Polish primary care. I suggest that this clash and the resulting pressure, together with recently more unpredictable state policy, are the main reasons of primary care practitioners unstable professional identity, difficulties in the patient-doctor relations, and, in some cases, a general disappointment of being a primary care practitioner.
